To eliminate health disparities and improve functional and clinical outcomes, High Plains has changed the way we deliver care. High Plains has special programs for patients who have diabetes, cardiovascular disease, asthma, depression or oral health needs. We follow specific clinical guidelines for treating these illnesses and contact our patients for necessary care and testing throughout the year. By being proactive, we can help patients better manage their chronic illness. For more information, go to Health Disparities. |
