ACCESSING Services

APPOINTMENTS

High Plains accepts new patients and all insurances.  We prefer to see our patients on an appointment basis, and we request that you call in advance so we can reserve time for you.  Same day appointments are available at all locations and phones are answered Monday-Friday at 7:45 a.m.  Routine appointments may be scheduled in advance.

EMERGENCY AND AFTER HOURS CARE

In the event of a severe situation, or one in which you are in doubt, go immediately to the emergency room of the nearest hospital.  Once there, let someone in the ER know you are an HPCHC patient.  If you need to talk to a provider after hours, please call 719-336-0261.  Your call will be answered by the answering service who will forward your message to the “on-call provider”.  Your call will be retuned at the earliest opportunity.  Please respect our providers’ privacy by not calling them at their private residence.

TELEPHONE CALLS TO PROVIDERS

You are encouraged to call with any questions you have concerning medical or dental problems.  If you do call, you will talk first to the medical or dental assistant.  The assistant is your link to the provider and they will do their best to answer your questions.

PRESCRIPTION REFILLS

All prescription refills should be requested at least 3-4 days before your prescription runs out.  Please call your pharmacy to request your refills, or if you get your medications at HPCHC Dispensary, please call 336-1211 or 336-5222.

PAYING FOR SERVICES

HPCHC bills all insurances. 

MEDICARE: If you are on Medicare, we waive your $100 deductible and offer a sliding fee scale for co-insurance.  If you have a supplemental insurance, we need the subscriber number, group number, insurance address and name of the policy holder. 

MEDICAID: If you have Medicaid, the Primary Care Provider will be verified at each visit.  If your card has another doctor listed as your PCP, please change your PCP to High Plains by calling the PCP Hotline at 1-800-221-3943. 

PRIVATE INSURANCE: If you have private insurance and it is a PPO, you must call your insurance company to change your designated provider to High Plains.

High Plains also offers programs that allow you to receive discounted services on an ability-to-pay basis.  (See other page: “Payments & Discounts”.)

CONNECTED CARE TELEHEALTH VISITS

Connected Care is a convenient way for patients of the High Plains Community Health Center to receive specialty care from specialists affiliated with the Centura Health network of care at St. Mary-Corwin Medical Center in Pueblo and St. Anthony Central Hospital or Little Adventist Hospital in Denver. Using high-definition audio and video technology, Connected Care delivers a telehealth experience remarkably similar to an in-person visit with a doctor.
