High Plains participates in many special programs to assist our patients in accessing affordable, quality health care and adopt healthy behaviors:CICP
High Plains accepts and registers patients for the Colorado Indigent Care Program. This program provides discounted health care services to low income individuals at participating providers. CICP is not a health insurance program. The program is funded with federal and state dollars to partially compensate participating providers who provide health care to the uninsured and underinsured. The Colorado Department of Health Care Policy and Financing administers CICP. Health care services are provided throughout the state by hospitals and clinics that participate in CICP. Not all hospitals and clinics participate in the program.HPC Sliding Fee Scale
High Plains Community Health Center also offers an in-house sliding fee scale to patients who may not meet all of the criteria to qualify for the CICP program. While CICP is accepted state wide, the HPC Discount program is only available to patients who access the care provided at any High Plains location. Covered services include medical, dental, behavioral health and dispensary.Wisewoman
High Plains Community Health Center is proud to offer our low income, un-insured or underinsured women a program that can reduce their risk of cardiovascular and other chronic diseases, through risk factor screening, referral services, and healthy behavior support options. The name of this program is WISEWOMAN (Well-Integrated Screening and Evaluation in Women Across the Nation). Women aged 30-64 may qualify for this life changing program and the great thing is it is tailored to each woman's needs. Some of the Healthy Behavior Support Options offered at our Clinics are Health Coaching, Diabetes Prevention Program, Cooking Matters, Tobacco Cessation assistance and other free classes throughout the year.One of the requirements for the Wisewoman Program is for women to be eligible for the Women's Wellness Connection Program through the CDPHE. This program provides uninsured or underinsured women that meet the income requirements a way to get their yearly women's wellness exams and mammograms. If you are between the ages of 21 to 39 we can help you schedule your women's wellness exams including cervical screenings. Women aged 40 to 64 can receive yearly mammograms along with their yearly exam. If you have insurance we can help you navigate the difficult task of scheduling these important wellness exams.

Women's Wellness Connection
Free cancer screenings for uninsured women:Age 40-49: Free pap, pelvic exam & breast exam
Age 50-64: Free mammogram, pap, pelvic & breast exam
Eligibility: Income & age eligible, uninsured or underinsured, one year since last pap or mammogram.
