COVID-19 (Coronavirus)


If you have a cough, sore throat, shortness of breath, headache, new loss of taste or smell, muscle pain, and/or repeated shaking with chills or have recently traveled outside of Prowers County or come into contact with someone who is confirmed to have COVID-19, please do not enter the building, please CALL 719.336.0261 FIRST and we will send staff to your vehicle or advise you of next steps.

COVID-19 Vaccinations:

We are currently vaccinating patients and non-patients age 18 and older using the Moderna vaccination. This vaccination is a 2 shot series taken about 28 days apart. It is recommended that both doses be received for maximum protection and vaccine efficiency.

Community Testing Location

High Plains is proud to be the Community Testing Location for Prowers County. We are testing community members, regardless of symptoms, Monday through Friday at 11:00 AM. If you are interested or think you may have symptoms related to COVID-19, please call Public Health to be placed on the testing schedule. Their number is 719.336.8721. If you are having severe symptoms, call 911 for immediate assistance.

Patient Information Update:


All patients, attendants, and staff who enter any High Plains buildings will be screened and have their temperature taken upon entrance.

High Plains has been closely following recommendations from the CDC and CDPHE to establish guidelines that will keep our staff and patients as safe as possible. Below is a list of our services and what is occurring with current patient scheduling.

Medical Services
Providers are seeing patients both in person and through telehealth services on the Healow app. Appointments can be during our walk-in times from 8-9 AM, same day appointments, and pre-scheduled appointments. Please call to schedule an appointment and a medical services staff will ask you screening questions to get you the appointment you need.

Diabetes Education and Nutrition Appointments
All visits are being scheduled in person and through telehealth through our Healow/Spruce apps. Please call 719.336.0261 to get set up.

Behavioral Health
Our Behavioral Health Clinicians are currently seeing patients in person and through Telehealth and the Healow/Spruce App. Appointments will vary upon the Behavioral Health Clinician you are scheduled to see. Staff will be available for warm handoffs during regular business hours, as needed. Contact 719.336.0261 to get set up.

Dental Services
Our Dental Department is currently seeing patients back on their regular schedule. We have done MULTIPLE updates to our building and processes to ensure it is the safest visit possible. Call today to get scheduled.

Please bring your own mask to your appointments in an effort to help us preserve our masks for staff and patients that do not have their own. If you do not have a mask, you will be provided with one during your visit. If you are an attendant at an appointment, you must bring your own mask.
